Job Description

  • Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.

  • The Community Care Navigator will provide assistance with coordination of access to health care services and provider referrals to low income uninsured or underinsured clients eligible for the Access Health/HOP program

  • Will help to facilitate access to health care by reducing or resolving barriers for assigned and/or referred clients.

  • Responsibilities of the Community Care Navigator include, but are not limited to, case coordination, patient education, travel/home visits, and direct follow-up related to accessing and/or retaining health care and supportive services.

  • Must use own car for travel to remote sites, often in rural, isolated areas with proof of current auto insurance

  • Ability and willingness to provide emotional support, encouragement, and motivation to patients

    Required Skills

Qualifications /Training:

  • Knowledge base of community services and resources

  • Must be able to handle potential problems related to safety issues for staff and patient in all settings.

  • Knowledge of medical terminology required.

  • Basic computer skills (Excel and Word)

  • Good communication skill, such as listening well, and using language appropriately


  • High School Diploma or GED

  • Willing to obtain BLS certification
