The Registered Nurse, Condition Management is responsible for improving the patient’s overall chronic disease management and well-being. This nurse will serve as a case manager, across multiple continuums of care, often needing to independently identify solution. S/he will act as an advocate for patients, collaborating with physicians, advanced practitioners, clinical staff and community resources utilizing a proactive, multidisciplinary team based approach directed toward prevention, education and health promotion.
This role is patient-facing and requires working in a clinical setting. The noise level is moderate and there is a potential for exposure to infectious diseases and blood-borne pathogens. It requires the ability to stand and sit for hours at a time (with some bending and stooping), ability to use manual dexterity in relation to clinical requirements, and ability to lift 50 lbs. Hours may vary based on location, patient volume, and business needs.
Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion
Delivers one on one and/or group coaching sessions in the areas of diabetes, asthma, weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health
Consults with other members of the healthcare team to solve problems and seek advice
Designs interventions to improve outcomes
Provides disease specific self-care education
Assists in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interprets study results, identifies limitations and barriers and recommends alternatives
Participates in outreach and follow-up as part of care management
Proper, concise and complete EMR documentation and/or paper based documentation for all assessments and procedures
Other duties as assigned such as new models of care and new types of procedures, etc.

Graduate of an accredited School of Nursing with current state licensure
4+ years dedicated nursing experience
Maintain BLS/CPR and CME/CEU’s as applicable for licensure
Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.
You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
Bachelor's degree (BSN) from four-year College or university
Working knowledge of computerized information systems (EMR) used in clinical management settings
Experience with condition management
