This position is located at the Richland County Health Department

Nurse Home Visitor for first time mothers enrolled in the Nurse-Family Partnership program

Nursing Process and Direct Client Care

  • Utilizes the Nursing Process to develop, coordinate, imp1ement and revise culturally competent nursing care plans with clients, families, communities and health care providers.

  • Develop plans of care that reflect assessment and/or population diagnostic findings and will demonstrate mutually agreed upon measurable, achievable goals. These goals will utilize appropriate resources and will be tailored to meet the client's needs

  • Utilize other team members, other agency programs and providers, and community resources to enable client to achieve setgoals

  • Demonstrates competency as evidenced by direct observations and documentation reviews

  • Maintains a minimum caseload of 25 clients

  • Completes all documentation, client care, paper and electronic documentaion within the mandated time frame and submits all documentation to administrative specialist within 1 business day of completion

  • Consults with team members, supervisor, and other health care providers as needed to ensure client needs are met

  • Promotes all Agency services as applicable to eligible clients

  • Establishes and maintains a working relationship with all providers involved in the client's care for the duration of participation in NFP

  • Report any issues and concerns regarding client to the appropriate provider immediately as they arise Wellness Promotion/Population Health Develop and tailor education plans according to assessment of individual/group/community needs

  • Develops, in conjunction with client, a specific plan of care based on the level of needs, community resources, family involvement and other health care providers involved in the client's care

  • Evaluates outcomes and adjusts goals as needed with input form client and other health care providers

  • Promotes and assists client in receiving other services through DHEC to promote health and well being such as WIC,IMM and PH

  • Shares findings, outcomes, successes and areas needing improvement with team members, community partners and other disciplines involved with the client and program

  • Promotes the importance of health, physical activity, preventive care and good decision making with each client served

  • Promotes value of education, child rearing, financial independence and growth with each client and assists with plans to meet these goals

  • Participates in surveillance, case finding and reporting with internal and external providers. Leadership

  • Exhibits leadership by participating in professional organizations, promotion of the NFP to internal and external providers, continuing education and serving as a role model to clients co-workers and the community

  • Identifies and shares with teammates, opportunities for professional growth to develop and maintain competencies

  • Maintains up to date information on all aspects of NFP, DHEC an the community regarding services available

  • Participates in committees, community partnerships, and promotional activities to promote growth and sustainability of the NFP and DHEC services

  • Seeks opportunities for growth and advancement within the Agency

  • Serves as a mentor/coach to new hires

  • Promotes teamwork and unity within the program and discusses all concerns/issues as they arise with direct supervisor

  • Demonstrates effective interpersonal skills and cultural competence in all interactions with clients, co-workers and partners

  • Functions as a public health team member in emergency preparedness and response situations in the Region/State as needed CQI

  • Participates in program and peer audits quarterly

  • Adheres to Agency and NPF/PFS policies, standards and regulations

  • Meets required continuing education hours each year

  • Provides input into the development and revisions of policies and procedures based on practice findings

  • Promotes the utilization of all available Agency resources as appropriate to clients served in the NFP program.

  • Completes billing and remits with visit documentation for all eligible clients. Pay for Success Study

  • Recruits and enrolls eligible participants into the Pay for Success Study per study protocol and in keeping with DHEC policy.

Minimum and Additional Requirements

State Minimum Requirements:Licensure as a registered nurse by the South Carolina Board of Nursing. Some positions require a valid driver's license. Also, some positions require certification in Basic Life Support and/or Basic/Advanced Cardiac Life Support. Graduation from an accredited school of nursing.

Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

DRIVING RECORD: If this position requires the applicant to possess a valid driver's license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
