We're looking for an enthusiastic Human Services Coordinator I for our Upstate Region who will provide an array of Coach the Family Engagement Child Family Team Meeting (CFTM) services as assigned. This role will be key implement the CFTM process with fidelity to the model region wide.
The Human Services Coordinator I provide an array of Family Engagement services through preparation, planning, and facilitating CFTM's.
Identify and assess the need for a CFTM and hold a Pre-CFTM meeting with the family to begin preparation for the CFTM and submit referral in Child and Adults Protective Services System (CAPSS).
Engage families in the family finding process to include Genograms, Ecomaps, Connectedness Charts, and CLEAR or Seneca Searches.
Document CFTM's accurately in CAPSS, completes IV-E, and other required documentation.
Increase community and provider network awareness of DSS Family Engagement practices such as child and family teamings.

Minimum and Additional Requirements
A Bachelor's Degree in Social Work, Social Welfare or Behavioral Science and one (1) year of professional experience in Human Services or Social Service programs.
A Bachelor's Degree in any other field and two (2) years of professional experience in Human Services or Social Service programs.
A Master's Degree in Social Work, Social Welfare or Behavioral Science.
A Master's Degree in any other field and one (1) year of professional experience in Human Services or Social Service programs.
Selected positions may prefer supervisory experience.
Must posses knowledge of clinical practice with children, adults, and families. Knowledge of human growth and development, social services, rapport building, and family centered models of intervention. Must have the ability to supervise, implement, and train on evidence-based family group decision making models. Must have the ability to monitor and develop outcome measures associated with desired program outcomes.
A Bachelor's or Master's degree in a Social Work or Social Welfare program accredited by the Council on accredited institution, plus experience as a professional Social Worker.
